Jared Walters is not credited in the book as editor, but is given on the copyright page as the copyright holder for the anthology, and is given as the editor in other sources (e.g. http://www.worldfantasy.org/awards/2002.html ).
Numbered pages run 8-325. The William Blake poems appear prior to the title page on unnumbered pages; art credits for these preliminary pages are given on the copyright page with roman numerals, so the page numbers are listed accordingly.

The titles with translations are exactly as printed.
The art portfolios images are credited as they would be in an art catalog, so they have the title on one line (for titled works); a few works explicitly say "Untitled", but others just have no title. Then there's a line with the year, medium and size, and then a line with the original publication information, or "Previously unpublished". For the untitled works, enough of this other information was added to the title to create distinct titles, which is why a few of the works have the size specified, and in some cases the page number was used to distinguish it from other titles.
