Of the 250, five have an introduction by Raymond A. Palmer, the rest have a different introduction by Lawrence A. Keating. [Currey]
"Daddy!" by Jack L. Chalker in Fantasy Newsletter No. 61, August 1983 claims that 6 copies of the book with Palmer's introduction were retained according to the publisher Conrad Ruppert and letters from the Milwaukee Fictioneers; Ruppert was still alive and publishing in 1983. Even at that time, only 5 copies could be accounted for.
Worldcat indicates that the publisher is not identified. Rupert is per Miller/Contento, though they also list it as Milwaukee Fictioneers in another context. Tuck lists his name in parentheses. Reginald identifies the publisher as "Milwaukee Fictioneers"
