Although not indicated anywhere in the book, Douglas Menville basically wrote the first half of the volume (1895-1959), and Robert Reginald the second half (1960-1977). Mary Burgess typeset the volume and did the index, and Reginald designed and laid out the interior text, including the black-and-white film stills.
