First published July and August 1888 in Harper's Magazine, vol 77, pp. 181-209 and 345-63, under the short title, with 10 illustrations by {{a|Thure de Thulstrup}}.
Four of the original illustrations are full-page, one as frontispiece of the July issue (p. 164). The artist is not credited; but some drawings are clearly signed "Thulstrup".

From Longmans (VH notes no other book publishers) the novel was first issued in 1888, under the long title, without illustrations (30,000 copies). The 1891 edition (New York and London; 10,000 copies) contained 8 full-page lithographs by {{a|Charles Kerr}}.

Library of Congress (LCCN, below) reports "157 p., [10] leaves of plates : ill. ; 19 cm."
Those must be plates of the 10 illustrations by Thulstrup for the serial edition of the story in Harper's New Monthly Magazine, whose conclusion was published only a week or two earlier, in the August issue. See the novel record.

Publication date is that of a notice in the present tense (earliest notice of book publication found), the first item in "Literary Notices", Hartford Courant 1888-07-31 p2:
"Harper & Brothers and Longmans, Green & Co. of New York each publish Rider Haggard's Maiwa's Revenge in paper cover. It has been printed in Harper's Monthly. ...
(Price 25 cents; for sale by Brown & Gross.)"
