From bound volume 119 viewed at HathiTrust: Following its frontispiece by Howard Pyle, the interior of this issue begins with the Kipling story, pp. 489-97, under the three-line heading in three sizes and fonts: The House Surgeon A Story in Two Parts * By Rudyard Kipling where * refers to a footnote statement of Kipling's 1909 copyright. That suggests a longer title of the story, "A House Surgeon: A Story in Two Parts". Its two full-page illustrations (492, 495) are "Drawn by F. Walter Taylor" and "Half-tone plate engraved by H. Leinroth", from the labels of the first one.
