The record represents two (front and back) of the various cover illustrations (4 as of 2016-09) selected from 13 half-page interior illustrations that accompany two lines of Tolkien's poem each.
They illustrate the journey of Bilbo Baggins, Elrond, Galadriel, Gildor and unnamed elves from Rivendell to the Grey Havens and on to the ultimate West, which is broadly the matter of the short poem.
